Care co-ordinator service

At Trent View Medical Practice we offer a Care Coordinator Service.

Your Care Coordinator at the Practice is Dawn MacDonald, she travels between all 3 of our surgeries and can be contacted via telephone or booked for a face to face meeting at a time and day to suit yourself (within the surgery normal working hours). You can discuss medical and non-medical issues in confidence, she will then be able to signpost you to the correct services or professional to suit your needs and requirements.

What do I do with this book?

Keep it handy, and let your loved ones know where and what it is.

Bring it with you to appointments with GPs, hospitals etc

Healthcare professionals can record information in it for you

Take it with you when you travel, you can refer to it or hand it over to other healthcare people if needed

Sometimes at this age we do need a little help remembering things. ‘This is me’ provides information about you at the time the document was completed. It can help health and social professionals build a better understanding of who you are.

‘This is me’ should be completed by you wherever possible or by someone who knows you the best when not possible. It should be updated as necessary. PLEASE INCLUDE KEY SAFE NUMBER IF THERE IS ONE.

The information provided in this booklet will help us support you in the best way we can.

It can be used in different ways to make life easier. For example, you can hand it over to your GP receptionist to fill in appointment details in the relevant section for you. To your Nurse, GP or HCA to fill in details or record any results you may need like BP etc.

If you should ever need an ambulance you can hand this over to the health care professional, they will then have basic but important details straight away.

Use it for your own piece of mind, so you know people will know what you want and need, to help you control and retain as much control of your mental and physical health as you can.

You can include as much or as little information as you feel comfortable with.
